The state of Pennsylvania has a campaign to raise awareness of the need for Long Term Care planning and is providing some great planning information on Long Term Living in PA. There are some special initiatives already underway.

Money Follows the Person (MFP) is a program to help provide the resources for someone currently living in an institution to allow them to move back into the community in a family member's home or an apartment of their own. There are several requirements including financial limits and that the person can either live on their own with all the necessary provided services or can live with a family when they can not live entirely on their own. The federal government will provide $1.4 billion to the 31 states participating in the MFP initiative.

Own Your Own Future is a program where Pennsylvania is partnering with the U.S. Dept of Health and Human Services to provide a free Long Term Planning Kit which contains an abundance of information and audio stories of the how-to-steps to start planning for your long term care needs. Take Charge Online is a great site with more information about how to begin planning for your long term care. It shows how the whole family needs to have open discussions to prepare for all the major health issues associated with end-of-life.
