What to do if your W-2 tax form is missing

If you don’t have your W-2 form recording wages from an employer, it’s time to look into it, the Internal Revenue Service said Feb. 8.

The first step is to contact your employer, the IRS said. If the form was not sent, ask your employer for it. If your employer says it has been sent, make sure the address your employer has for you is accurate.

If contacting your employer doesn’t help, contact the IRS. After Feb. 14, the IRS will begin looking into missing W-2 forms.

After that date, call the IRS at (800) 829-1040. Have your full name, address, phone number and Social Security number ready to give the agency.

Also be able to provide your employer’s name, date and telephone number, the dates during the year you were employed, the amount you were paid and the total taxes withheld. That information is available on the final paycheck stub you received in 2012.

Whether you get your W-2 form or not, file your taxes by April 15. If your W-2 has not arrived, file Form 4852 instead. If you receive your W-2 after you file, you will need to amend your tax return using Form 1040X.
