The Pike County Public Library is joining the U.S. Department of State in celebrating Passport Day in the USA 2012, on Saturday, March 10th from 10 am to 3 pm at the Milford Branch, located at 201 Broad Street. Passport Day is a national passport acceptance and outreach event. PCPL is hosting this special passport event to provide passport information to U.S. citizens and to accept passport applications.
Two separate checks or money orders are needed for the application fees. The application fee should be made out to the Department of State. And the second, an execution fee which is required by Federal law, should be made out to Pike County Public Library – as the processing agent.
Fees for an adult passport and card – 16 years and older – are $140 (plus $25 execution fee payable to PCPL) for first time applications. For just the adult passport, $110 (plus the $25 fee payable to PCPL). And for just the card (which can only be used when going to Canada or Mexico but not for flying in / out of the country) $30 (plus the $25 fee for PCPL). Payment will be accepted by the Pike County Public Library employees, who are certified agents of the U.S. Department of State.
Fees for minors (16 years and younger) are $95 plus $25 for the Passport and card, $80 plus $25 for the Passport only and $15 plus $25 for the card only.
All forms will be available at the Milford Branch on Passport day, as well as expert assistance by Pike County Library staff. (Including Form DS-3053: Statement of Consent - Issuing a Passport to a Minor Under Age 16, which is necessary for divorced parents seeking a passport for their minor child.)
Documentation required to apply for a passport are:

Proof of citizenship (previously issued and undamaged U.S. Passport, Certified birth certificate issued by the city, county or state (note – some short versions may not be acceptable documentation), Consular Report of Birth Abroad or Certification of Birth, Naturalization Certificate, Certificate of Citizenship).

Present Identification (previously issued and undamaged U.S. Passport, valid driver's license, naturalization certificate, current government identification, current military identification)

Passport Photo (color, 2x2, head to chin is between 1 inch to 1 3/8 inch, full frontal face view, neutral expression with both eyes open, in front of a white background, taken within the last 6 months, non glare glasses, and wearing clothing you normally wear – no uniforms but daily worn religious clothing okay)

You must have a valid passport if you are planning to travel outside of the county and are entering or re-entering the United States by air. When entering the United States from Canada, Mexico, the Caribbean and Bermuda at land borders and sea ports of entry, U.S. citizens must present a passport book, passport card, or other travel documents approved by the U.S. government.
Appointments at are not necessary on Passport Day, but may be made in advance calling 570-296-8211. Pike County Public Library has passport processing available throughout the year, but an appointment must be made to ensure certified staff members will be on duty. Both PCPL employees will be available at the Passport Day event.
