'Grey's Anatomy' 10x21 spoilers: Jackson has bad news, Alex lectures Arizona

In this week’s episode of “Grey’s Anatomy” titled "Change of Heart," there seems to be a lot going on, but when isn’t there? It is the first of the four final episodes with Sandra Oh as Cristina Yang and ABC is pulling out all the stops to promote these last few episodes. Promos and sneak peeks are popping up all over, like the one shared on Latin Post on April 21.

To recap last week’s episode, "Go It Alone," after struggling to write her acceptance speech and forbidding anyone to accompany her to the ceremony. Once there, however, she reconsiders and calls Meredith Grey to invite her. When she enters the banquet room, Meredith and Owen Hunt are already there. Sadly, Cristina learned she didn’t win the Harper Avery Award.

Meredith and husband Derek Shepherd are having problems juggling kids and careers. Last minute surgeries put them both in the OR. Meredith is in a panic when she can’t find the kids and Derek isn’t returning her calls. When the storm dies down, the couple agrees something’s got to give.

Callie and Arizona have a discussion about which of them will get pregnant. When they can’t decide, they agree to a coin toss. At the last minute, Arizona tells Callie that she should be the one to carry the baby and Callie is thrilled.

The honeymoon is over for April and Jackson as a major argument erupts over their differing religious beliefs. When Jackson admits he doesn’t believe God exists, April says she can only pity him. She ends up crashing at Callie and Arizona’s place.

The synopsis for “Change of Heart” reads:

"Change of Heart," "The doctors receive bad news from Jackson. Meanwhile, Amelia arrives in Seattle for an unexpected visit; Richard heads to Boston to surprise Catherine; and Bailey makes a bold decision after growing disappointed with the lack of progress in her research."

Not mentioned in that synopsis, though is what goes on between Alex and Arizona. Wetpaint shared the scoop and another plane trip is involved.

In the sneak peek, the two pediatricians are on a charter plane and it stirs painful memories. Arizona begins reminiscing about the last time they shared a plane ride. Alex mistakenly thinks she means the time she kicked him off a plane because she was mad that he was considering a job at Johns Hopkins and went in his place. That time the plane crashed, 2 doctors died and Arizona lost her leg. He turned the job down and stayed for Arizona.

What she was really referring to was when they first met. They both flew out to get a liver and small bowel for a 10-year-old patient. Still Alex is right in thinking she wants to talk him out of taking the job at the private practice. He interrupts and tells her to mind her own business.

"I know you think I'm selling out — that I'm choosing money over substance and that it's a waste of my talent, or whatever," he rants. "That there's more to life than nice schedules and a big fat paycheck. Well, I thought about it hard. You've been awesome, but I've already stayed here once for you. I have to start thinking about myself, about what I want. I worked hard, and I deserve this. I deserve to think about myself for once, OK?"

“Change of Heart” airs on “Grey’s Anatomy” on Thursday, April 24 at 8 p.m. on ABC.
