The Utah Department of Health Immunization website states that there is no charge for H1N1 vaccine, however a small fee may be required for giving the vaccine.
As of today October 19, 2009 here are a few places in Salt Lake City that have the H1H1 shot:

The regular flu shot can be given at Kmart and Smith Pharmacy’s, Intermountain Health Care locations, in addition to the clinic’s listed above.
