Demi Moore was rushed to the hospital Monday night for an issue with substance abuse.
Law enforcement received a 911 call at 10:45 Monday night. When paramedics arrived at Demi's Los Angeles home, they assessed her and transported her to a local hospital.
Sources close to the actress say she's being placed in a facility to "seek further professional assistance" for substance abuse.
Demi's representative admits she's getting the professional help she needs.
Because of the stresses in her life right now, Demi has chosen to seek professional assistance to treat her exhaustion and improve her overall health. She looks forward to getting well and is grateful for the support of her family and friends.
