Case managers for seniors in Nassau County (Video)

If your family has a senior person having difficulty managing independently, you may be relieved to know that Nassau County provides case management assistance.

Through the Nassau County Office for the Aging, a case manager can assess the situation and evaluate the senior citizen's needs. All appropriate services and resources are discussed and investigated. This process can include a financial assessment to determine the person's share in the cost of any services. A detailed written care plan is reviewed with the family before any services take place.

The case manager can also determine the eligibility for in-home aide and ancillary services based on the following criteria. The person must be 60 years or older; require the assistance of another person for eating, bathing, dressing, toileting or transferring; or in two of the following: housekeeping, shopping, preparing meals, managing money, laundry, or using transportation services. The person must have an unmet need and not be receiving similar assistance from any other source; apply for Medicaid if eligible; and be able to live safely at home.
