BP oil spill update: Read BP findings from internal investigation (video, documents)
BP announced the publication of their findings from their internal investigation regarding the explosion of the Deepwater Horizon oil rig on April 20, 2010 and subsequent catastrophic oil spill from the Macondo well. The BP oil spill internal investigation was led by Mark Bly who serves as Head of Safety and Operations for BP and instituted a team of more than 50 specialists.
announced that there was no one, single factor that led to the tragedy, but rather a series of failures from different parties that all contributed. The video released by BP is a summary of the findings from the investigation. BP said that their mandate was to identify critical factors to prevent similar accidents from happening in the future. You may read the report below and view the video to the left.
