Authorities are investigating the circumstances surrounding the death of a man who died Wednesday night after D.C. emergency responders went to the wrong address, D.C. Fire Chief Dennis Rubin said.

The delays caused the ambulance to take more than 34 minutes from the moment the 911 call went out until the time the patient was transported to Howard University Hospital. Rubin called the delay an "operational error" and has ordered a review of the incident.

Crews were dispatched to the front of 10 G St. NE for a call of an adult male having a seizure, but were unable to locate the patient, Rubin said. The crews searched the block for about two minutes, and dispatchers tried to reach the caller but got no answer. The rescue crews started back to the engine house.

Two minutes later, crews were dispatched to the site again, Rubin said. Rescue workers realized that they had been at G Place, one block over from G Street, Rubin said.

Man dies in D.C. double-shooting

D.C. police are investigating a double-shooting in Southeast Washington that left one man dead.

Police found a man suffering from gunshot wounds inside a car that was stopped in the middle of the 800 block of Chesapeake Street at about 6 p.m. Thursday. A female victim was lying in the street alongside the car.

The man, identified as Jermaine Chambers, 26, of the 800 block of Chesapeake Street Southeast, was taken to Prince George's County Hospital where he was pronounced dead.

The female victim, whose identity is being withheld because she is considered to be a witness in the case, was transported to an area hospital and admitted in critical condition.

Police investigate homicide in Reston

Fairfax County police are investigating as a homicide the death of a 42-year-old Reston woman who was found unresponsive in her home Thursday night.

Officers discovered Zaida Alvarez Rodriguez at 11:30 p.m. while responding to an "unknown situation" at the Greywing Square residence, according to a police statement.

The initial cause of death appears to be trauma to the upper body, though authorities are still waiting for the results of an autopsy. A police spokesman declined to comment further on the case Friday.

Pedestrian killed in Dumfries

A 21-year-old woman was struck by two cars and killed trying to cross Dumfries Road at 11 p.m. Thursday night in Prince William County, police said.

Jeanna Elizabeth Stewardson, 21, was killed at the scene, police spokeswoman Officer Erika Hernandez said. Stewardson was wearing dark clothing at the time of the accident, which did not occur in an intersection or crosswalk, Hernandez said.

The drivers of the two vehicles, a 2002 Mazda Prot‚g‚ and 2007 Mazda 6, were not speeding or under the influence of alcohol and no charges were filed, Hernandez said.
